Squarespace Form with Google Places Autocomplete API

I am looking for a freelancer to help me with my project. The skills required are Javascript, Website Design, HTML, CSS, Google Maps API and Squarespace. I am happy to pay a fixed priced and my maximum budget is $30 USD. ---- Create a user input form for Squarespace code block. Form should contain the following input prompts: Address* Property Type Sale Type First and last name Email Address Cellphone Number *Should use Google Places Autocomplete All inputs must be emailed back to us for processing. Must be able to inject into Squarespace code block via iframe or script Must be mobile responsive and able to work within iframe without distorting on mobile Will need a populate a successful entry pop-up Video can be attached for visual reference.
