A survey tool in Salesforce

A survey needs to be sent to customers from SFDC. The survey needs to ask a mixture of questions and collect responses from picklists, multi select and free text. It would be good to be able to include pictures on the form that the customer fills out but this is not essential. The survey page should not display the usual SFDC banner up the top (if possible) and should be in keeping with our brand guidelines. The code should be fairly well notated for easy changes in the future. An email will be sent from the custom object that we use to the customer who is a defined contact in the custom object (using the SFDC email function which is already configured). The results will then show as a related record to the custom object. I have a basic understanding of apex but not enough to complete this task without the use of third party applications which is not desirable due to the nature of the data we handle
