Web Form Project

Hi all, I have a folder on a website with nothing in it (yet!) that I'll disclose to the winning bidder. I'll need you to create the following on that site: 1. HTML Form 2. Submit email address (must be validated to a particular companies email) 3. Form to submit URL 4. The root domain of the URL (as submitted in 3) is then checked against PageRank with a deduction made from the page rank value for every directory the URL is away from the root domain. - For example a URL has a PageRank of 5, but the page resides two folders down, so the PageRank is 3. 5. Form to submit twitter handle of tweet that contains client URL 6. A score is then calculated from the number of followers from the twitter handle 7. Both the values would be assigned to the email address in a database. 8. The score would then be presented to the user. I'll need a really simple skin for this form in line with a brand, that I'll send to you.
