Online calculator tool

Hi Freelancers, I have a Excel spreadsheet that needs to be converted into a PHP application. The spreadsheet asks a user to enter in multiple lines of data and select from some drop down boxes of variables in order to get a calculated response/ The spreadsheet contains all of the formulas and logic that is required. It is unlocked so you can see everything. The application will calculate energy usage and possible savings based on what the user enters. The user should be able to enter as many lines of data as they like, so the application needs to be able to deal with this. Upon completion of the input data, the user is given a calculation of their data inputs and a recommendation based on the logic in the spreadsheet. They need to be able to download a PDF report of their results, which also contains a second page of non-changeable promotional information. The application should collect some of their personal details via a form, and save their details, data inputs and the calculated responses to a MySQL database for reporting purposes (reporting not required in this project). Screenshots attached show: 1: Collect Personal Details 2: Data entry of electrical items and their daily use (only the first 3 rows are data entry, rest are calculated.) 3. Action Plan - Selection of a cost saving method for the electrical items selected. Each field under 'energy saving options' is a pre-populated drop down box 4. Report shown on screen and available as a PDF download. I'm looking for a nice clean Web2.0 style tool, so you are free to take some liberties with layout and design.
