Healthcare Software Project

i have a healthcare website and want to build a patient portal in the back of it. I will need a dedicated team that can work the extensive project from start to finish. I need the team to be able to send me daily updates and my goal is to have the project completed and ready to go by March 1st. 2014. Please don't bid on this project if you are not capable of communicating daily to the status of the current work. The project includes web design, database design, automated emails, text messaging, patient reminders, alerts, patient calendars etc...the features needed to allow a doctors office to manage patient care. I will separate this project into dozens of parts beginning with Phase 1 of the project and here are the details. Phase 1. Design the patient page as shown. I currently have only the blue header and white page designed and incorporated in my site. The patient table does not exist. I need you to design the patient table to be able to list the patients as shown. The "Add Patient" link creates a popup that allows me to enter patient data to allow the patient record to be displayed as shown. Please see the "Add Patient" .pdf attached. I want the page to have the SAME soft look and feel as the images. The page is to be designed in php/mysql/ajax and other modern software applications. Please bid on Phase 1, designing the layout and table and incorporating the "Add Patient" feature. If you are good and are a frequent communicator the project is yours. Phase 2 (the next project) will be displaying the patient record and medical history.
