I have a paper spreadsheet that we use daily to capture shipment information. ? There are only 11 columns on the sheet. ? I want to create a database to hold the information with a web based data entry page. ? One of the fields (carrier) needs to have it's own table to store carriers and the data entry field needs to actively look up previous carriers as it's being typed. Along with the data entry page, I also need a report page to show the data in a spreadsheet view based on date. ? Some of the fields on the existing sheet are choices to the same question, so can be combined in the database.
