Mobile app - Offline Work Permit Writing, Signing, and Emailing app

I need a mobile app designed as an "ease of use" .PDF filling program to replace paper form permits in an area with minimal to no cell service. • Fill out an established fillable .pdf form (Work Permit), in an idiot proof fashion, broken down by numbered sections. • Be able to concurrently manage up to 6 different forms (additional types of permits added to the main permit) with the final closeout combining and saving all as a single .pdf. • Generate a unique 6-digit permit number (E123456UserInitials) and assign it to a permit bundle. • Integrated with Microsoft Outlook; Auto email a copy of the .PDF when permit first "Opened" and 2nd email copy when "Closed" to multiple email addresses (self, supervisor, safety guy) • Be able to transpose certain common filled boxes (Location, Date, Company, etc.) automatically between different permit types to reduce repetitive typing of the same information. • Not require an online database or login management, essentially an "ease of use" document filler with cache capabilities in the event of no service and will auto email completed permits once service is reestablished. • Use a card-based UI (or best available option) to organize multiple signature boxes for multiple people to sign once all permits are filled. • Store completed permit bundles for field referral as needed. • Several other small nitpick requirements.
