I have several patient demographic and background forms for a medical private practice. Instead of having clients print out the PDF, hand write the answers, and deliver them to me I want to: 1. Allow the client to complete the forms on a keyboard or tablet, with the text in the response boxes being easy for them to see and review (i.e. not having anything that they write in their responses disappear from view). 2. Have this file be easily encrypted so the client feels secure in sending it back to me via email (although my email is HIPAA compliant, it is not encrypted, thus I need to protect the incoming files). 3. Have some way to easily and automatically import the completed responses into an Excel file; and if possible, a Word Template 4. The end result is that I can quickly and easily transfer the client's background information into various reports that I write that require this information. 5. I am sure I could figure this out, but hate to spend the hours messing around with Adobe to learn myself.

Insurance companies like to collect a lot of information to evaluate risks and it is paramount that forms are easy to fill, easy to save, easy to send back and finally, easy to extract into Excel or a database. I have done a several complete interactive sales aid for a clients as PDFs and they are all working beautifully and they love it. From my experience in this kind of tasks, the main problem with creating forms and other documentation for a client to fill out is that most are created in Microsoft Word, or other form of word processors and we’re afraid the client won’t have the right software to open, add to it and return it. In doing so, the design is compromised because of word processor’s inability to function like professional design software. While MS Word is typically found on most computers and regarded as the most compatible document to send someone to fill out and return, that doesn’t mean you can’t have your cake ( “good design capabilities”) and eat it too ( “compatibility). Way out Adobe Acrobat Professional gives the ability to turn your previously designed, standard PDF file into an interactive form that clients can download, easily fill out, print or effortlessly return to you by email. All they need is the free Adobe Acrobat Reader, which can be easily downloaded.
