Event Registration system in Google forms

I need an even registration system in Google Forms for an upcoming event I’m hosting. There are two categories within this event: TEAM and SINGLES There are two categories within each Team and Singles events: Boys Team, Girls Team / Boys Singles, Girls Singles Ideally, I would like to replicate the structure of our current entry forms (attachments) in Google Forms. I have included WORD docs of each Team and Singles entry forms. You can copy and paste content. A basic description of the registration process. We must have an exportable Excel file of all applicants in each of the four categories. I can describe what the exported Excel should display.
