Smart Data Collection Form

I am currently collecting data on a lot of pieces of equipment. I use a form and fill in a bunch of fields. If I have a duplicate piece of equipment I photo copy it and make the necessary changes. What I would like if a form that I use on a scribbler or similar computer. Some fields require drop down picks and some fields will fill in based on other picks anss some fields are just entry fields. Each form represents an asset and will have a unique number. Once the form is filled in it can be saved or recalled for edits. The form must also allow the user to search for an existing asset from as a start for a new asset. The data will be stored and recalled from a CSV file on the computer. The program should allow for changes to the pull down fields and to the inter-related fields. Highlighting of some fields would be nice to help walk person using form to do it right. All this info will eventually go into a CMMS database so no need for data redundancy and will consider MS office based options. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform to be run in windows
