Convert MS Word to MS Access 2003 Form

I have a 2-page business form (manual work order) developed in MS Word 2003. I need it transferred exactly into an MS Access 2003 form so that the printed form is identical when printed from either application. There are no linked database fields - I will add them later. This project is simply to transcribe the tables, boxes, lines and text from Word into Access. There are many fields and sub-forms to be added later so the simpler the design the better. For example, the use of boxes instead of simple lines to draw tables or the use of a single text box with several lines of text instead of several text boxes. This form will be printed double sided on a duplex printer so page 1 should be placed at the very top of the form and all the page 2 content should align as closely as possible with the bottom of the page (0.25in margins) to give as much space as possible at the bottom of page 1 and the top of page 2 where bar codes and other fields will be placed later. The Access form must not force a third page to be printed. ## Deliverables 1) All deliverables will be considered "work made for hire" under Canadian Copyright law. Employer will receive exclusive and complete copyrights to all work purchased. (No 3rd party components unless all copyright ramifications are explained AND AGREED TO by the employer on the site per the worker's Worker Legal Agreement). 2) Successful completion will be 90% based on the visual similarity between the printed Word document (supplied) and the Access form loaded into and printed from an empty MS Access 2003 database. 3) The simplicity of the design is 10% of the project.
