I need to fix this bug:
Query Error :: INSERT INTO surveys(name, description, begin, end, active, state, name_button, menu_left_pos, dashboard, graphics, createdby, created by_name, date_creation) VALUES ('does it work today?','','0000-00-00 00:00:00','0000-00-00 00:00:00','1','2','oct 10','0','0','0','0','', '2014-10-10 02:28:13').
It comes up when I try to create a form on my newly developed form builder application.

I've reviewed the error you provided and assuming that is the exact insert command your application is trying to use, the error seems quite obvious, (its in the 12th field), so with access to the code it shouldn't take more than a couple minutes to fix.
