My father needs to know what a " do not resuscitate" form is and also what a "Power of Attorney" is. I am his daughter. My father lives with me and I am his care giver for the past 3 years since he was diagnosed. I want to be able to make financial and medical decisions for him once he is not mentally capable of doing it himself.
