I am exploring ways to ask a series of complex questions in a user friendly way, I have found a way to do this but I need some help to modify the code to meet my requirements. Please look at the demo on [login to view URL], I have also attached the source files for convenience.
I would like to do the following:
1) Include a 'Back' button in the same style as the 'Continue' button, this should be greyed out on the first question, unlike the 'Continue' button it should not be assigned to any keys on the keyboard, the button should be fully responsive and appear in a similar way to the 'Continue' button when viewed on smaller screens (iPhones etc.);
2) At the moment I can use radio boxes, text fields and text areas as these have been styled, I want to use <select> elements too so I need the appropriate CSS styles written that mimic the style I am using with the other elements;
3) I would like to create a tooltip over some of the words of the question to give definitions etc, I would like you to style a pop-up tool tip in keeping with the overall design of the application that I can easily copy and use throughout the document. For example, on the first question I would like to define the word 'corporate' as "any legal entity, trust or partnership as defined in section 12.9.4 of the Companies (Bailiwick of Jersey) Law, 1999."
4) When the form is submitted, I need the answers given to populate fields in an existing PDF template and tell the user to download the completed PDF, sign it and return to the following address: "PO Box 248, St Peter Port, Guernsey GY1 1HB". I will be using this with different templates so I will need to have clear instructions on how to change your code. I have attached an example PDF, this is only a proof of concept so I don't expect you to complete every field in the PDF, maybe do 2-3 as examples (including a multiline text area).
5) Fix styling issues i.e. overlapping text fields with buttons, a better way to display dots at the side (when there are lots of questions, the dots go all the way down the screen and cause styling issues). I am happy with the numbers in the top right hand corner.
