Write some Software

We need a form (html) with php - YII (backend) for the next flow:

User:

1-. The user enter to a specific url

2-. Fill the next fields:

a-. Agency (select)

b-. Client (select)

c-. Campaign (Text)

d-. Username (select)

3-. Fill the number of item the user want with the next fields (like a "shop cart")

a-. Weight (select)

b-. Height (select)

c-. Type (select)

d-. Media (select)

e-. Creative Name (text)

e-. Type (Select)

* the user can add as many as he wants

4-. When the user click on create button the system must:

System:

Jira (atlassian)

1-. The system must create a new project (Jira API) with the following configuration:

a-. Project name must be like: Agency/Client/Campaign

b-. The type of the project must be copied from a template

c-. The system must create a Task inside the Jira project for every item on the "shop cart"

1-. The name of the issue must be like this: CreativeName/Type/WeightXHeight

2-. The system must include the tags: Banner and "Type"

3-. The system must assign a owner for the task (username)

4-. The system must assign the issue to the status To Do

Bitbucket

The system must create a repo (Bitbucket API) with the following configuration:

a-. Repo name must be like: Agency/Client/Campaign

b-. The project must be Agency

c-. The system must do the first commit with the following structure:

1-. JIRA ISSUE ID

Grunt Build + Deploy

Create [login to view URL] to build and deploy the git code to an FTP, the credentials should be configured on the Yii DB.
