Magento Custom Form - Save in db and email

Hi All,
I want custom form to add in Magento, which will store data in db and send email too administrator.
Following fields needed
1. Name [textbox]
2. Email [textbox - with email validation]
3. Address [textarea - max 100 chars validation]
4. Gender [radio button]
5. Hobby [checkbox- hobby1, hobby2, hobby3]
6. Favorite team [dropdown - list any sports team]
7. Submit button [on submit validate for all the above fields, all fields are required fields]
All the data should be store in db and email to administrator.
Note: This is pilot project for the right developer. If I am satisfied then I can give more work and bonus.
