Modify an existing PHP form

I have an existing functional PHP form but it looks very boring and unprofessional. I'd like someone with a graphic design background to layout the page in a more professional manner.
This won't involve changing the sequence of fields. It is just a matter of esthetics: fonts, alignment, section titles, etc. The form has about 15 fields. The user's selection of some options affects the number and type of later options - so the size of the form varies in real time and the layout needs to take that into account.
It is very important, however, that these cosmetic changes do not affect the functionality of the form.
Submission of the form also generates a confirmation email. I need a few minor changes to be made to the contents of that email.
