Build a Website with SQL or Access database

Hi, we need a great looking lead gen website with leads going into an online database, either in SQL or in MS Access. We need to be able to download the leads into an xcel file to re-upload into our main database. We have a model site, but are open to all suggestions.
We would like the site to be in Wordpress. Hosting is with GoDaddy. We will use Google Analytics. The site colors, fonts, pictures and logo will be similar to the main site which we will provide. We will provide you a word document with text and graphics. The site will have a main page, a privacy policy page and a successful confirmation page.
Please provide at least 3 work samples. We expect excellent work and great communication along the way. If this works out there will be future work.
