ASP and Access to MySQL

I have a very simple two page ASP site that uses an Access DB. I need to have this converted to a MySQL database as I will exceed the record entries one I start this page running.
The database structure is simply a record that contains 5 first names, last name, address, city, state, zip, email, comments. There are 40 fields for each record and a new record is created each time a someone accesses the site. If the site is completely written in PHP (preferrably) I would also need a basic "guest book" page as well.
I do not know what is entailed to access and utilize this as a MySQL DB, but I need a little help reworking the two pages so that it will function.
There is not currently any data to import in the DB as that will be created when the site is placed on-line.
I may also need a place server to support this site and I'll pay for that as well.
I simply need help getting it running and operational with the MySQL database.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
