PDF to allow data entry / tabbing / saving

I already have a PDF (see attached) which at present is printed and filled in by hand.
I want this to be electronic, so data can be entered into the document into the individual fields and saved again with all the data in it. Should be a fairly simple fix.
- Fields need to accept text and numbers
- Need text to be centered in the small single character boxes
- All fields need to be tabbed from one to the next.

- Must be able to communicate via Skype.
