I created a detailed survey/analysis in Google Forms for my clients and potential clients to complete. However, I discovered that many of my customers are in healthcare fields and facilities who have servers that limit their ability to use Google Forms or Survey Monkey.
I initially wrote survey questions in a Microsoft Word document so I have the questions on that but I also exported the Google Form into a PDF document with all the images that I had in the Google Form.
I'd now like the Google Form converted into a survey/document (PDF?) with data fields that can be completed by my clients who cannot use Google Forms. The data fields need to include checkboxes, radiofields, free-text and dropdowns.
I need to be able to email the document to my customers for them to complete and they email it back to me once they are finished.
I need to be able to make updates and changes to the document on my own as needed.
I'd like some or most of the pictures included in the document and for it to look very professional and visually appealing.
