I have a form that needs changing slightly. I will upload the new form to my website for customers to fill out. I also need to show them an example of a form that has been filled out as another download. The form will be located at https://framelessglasspoolfencing.com.au under the heading "Get a glass pool fence quote by tomorrow" if you go to the url it should make sense.

1. you will need to change the look of the site plan form to include the fields that i have illustrated in "New Form". The new site plan should be the same colours and look as the "Current Form"
2. In another pdf file You will need to fill out the new form as my customer had filled it out then attach some photos to the same pdf. Like the upload i did called "example site plan" and the photos i uploaded
