Just the first two screens you've described. Although you've given us creative freedom with the other two screens - it would still be important to know what information would be on this screen in order to define the layout and weights for the different elements on the screen. This type of design is exactly what I'm looking for. I think for the Contacts page, it would look similar to a standard Contacts page (alphabetical order), but it should also show that person's bday on the right of their name. The Profile page can be generic, I would suggest including standard settings menus for now like Profile, User Birthday (ie, whoever is using the app), notifications (where they can setup notifications) etc.
