Database Management Project with Merge

We are looking for an online Database Entry tool with the ability to update the database, do a merge print to Microsoft Word (you will also be responsible for sample form) with the specific information in specific fields. We want to be able to run reports, add and delete users, we want to be able to add fields. Some fields will be text, checkboxes, dates. The database design needs to be documented along with the process to update the Microsoft word macro. The word application will be run locally. This will be hosted on a Linux webserver.
