A Wordpress Order Page (ONLY EUROPEAN FREELANCERS)

Hi,

Im looking for an order page in wordpress, what should be done.

1. A suitable graphic design of the page so it suits the rest of the sites design.

2. Make an order form with questions and fields.

3. Make a price plugin? How it works...

- You input sqm (example 44) and you get a showed price for the service

(the price should be inserted in back end like 0-10 sqm = 10 usd, 11-20 sqm = 20 usd etc.

- And below the customer can choose different extra options like

-- Cleaning windows on balcony

-- Cleaning floor of balcony

(and each option has its price that is added to the total in the end where price will show)

4. Input of name, adress etc.

5. Questions like

- Will the customer be on sight?

- Do we need to come and get key from your office?

5. A field where the total price shows.

6. A button to send the order.

7. The confirmation email with the information will be sent to 2 different email addresses and also a confirmation to the customer that the order has been received and waiting for confirmation.

This is basically it more or less and it has to be done off site and when everything works fine then need to implement on the live site.

Not such a big project but need someone that knows his way around wordpress and plugin but as well graphic/design so it looks great.
