Simple PHP + MySQL CRUD

I need to make a simple interface for a client to update a database with 3 tables (Companies, Projects, Groups). There will be 4 main pages as well in CSS + HTML which can be very basic and minimal but with some clean styling so it's easy to ready and use. Page 1 (Main Overview) - Show companies with a button to go to the page to view/edit company - button to add company - Show groups with a button to go to the page to view/edit groups - button to add group Page 2 (Company view/edit) - if it's new, provide a form with fields to generate new company - if it's view/edit of an existing entry show the same form populated with the database entries and also search the projects table and display a list of matching projects. Also show a button to add new project. Page 3 (Group view/edit) - if it's new, provide a form with fields to generate new group - if it's view/edit of an existing entry show the same form populated with the database entries and also search the projects table and display a list of matching projects. Also show buttons beside each entry to view/edit the company and view/edit the project Page 4 (Project view/edit) - if it's new, provide a form with fields to generate new project - if it's view/edit of existing entry just populate the form with the data from the database and let the user edit it. Projects are always linked to a company, and can optionally put in a group. Simple project. I prefer procedural programming style and coding in PHP. No pre-made frameworks please but you may use a template for the GUI if you want. I want to see your coding style. If we like your coding we will keep you on board and give you more work.
