Simple Contact form with States

I require a simple contact form to be built, we will have a section in the form that contains a drop down list of 8-15 products. These will have a name and corresponding picture that should be added through admin as well as description. If the drop down changes from product a to c , then picture should also change. We also require links for each product through admin, so if we book product a, from product a page, then the contact form has product a selected. so something like contact-us.php-product=a which just forces the state. We will then link each product to the booking form. I have all the fields for the winning bidder and need it done quick.
