Script from PDF to Excel (or CVS)

I have attached a sample file. The customers name, address, telephone number and check in date. (and email if listed.
These files are in PDF and when just simple covert to excel the file because of merged cells and such does not covert in a way that is easy to pull this data;.
I am looking for someone that can run a script (coded) that will pull the info needed out of these files.

Actually this type of problem are quite easy to fix in fact I can run a code to have dome in no time if you want to have it as a cvs file or xls
