PHP script for contact form submission

We have a static website [login to view URL] hosted on a linux server. We want a simple script to be integrated with the website such that when a user submits the contact form, the contents should be emailed to the website admin. Please note that this is a US server and sends out emails only when the from email is of the domain name itself. Thus, the from email has to be from info@[login to view URL] Since this is a simple project, the price is fixed at Rs. 750/- and is non negotiable. Bid only if this is acceptable to you and you can complete the project.
