Online Timesheets Email as PDF x 2

Do not bid on this project unless you have read this brief and understand the project. I require 2 x forms to be created and able to be filled out on a wordpress site. There will be 1 x weekly timesheet and 1 x bi-monthly timesheet. To be built in wordpress and there should be the ability to alter the form / add new forms in the backend. Staff should be able to log visit our site [login to view URL] and login with a user name and password. This will be a common username and password. Once they login, there will be a webpage that you have created as part of this project. The webpage will be basic and just contain our logo, some text, and 2 buttons - weekly and bi-monthlytimesheet. There should be the ability to add more buttons / links and edit the page in the wordpress backend. The user will then click either timesheet, and another page will open that mimics the timesheets attached to this brief. Users shall be able to fill out this form and then submit. The form will then be EMAILED as PDF to the MANAGERS Name that has been selected, and also to the EMPLOYER. Therefore there must be the ability for us to add new employeers and managers in the backend along with their email address. Fields shall have the following functionality: Employee Name - Selected via drop down box Pay Period Ending - Selected via calendar popup. ie select the date from calendar Leave Type sections - select multiple dates from a popup calender. Ie if they select 2 dates, these dates will be listed under 'dates' and then the % time will fill out appropriately . 1 day = 9 % Job Number / Name - Enter text manually % time per pay - enter this manually TOTAL - to calculate automatically Form will not submit unless total = 100% - pop up box to notify user if does not equal 100% Managers Name - select manager from drop down box Date - Automatically fill with todays date Submit Button - Reset Button at bottom When submit - pop up box notifying form has been submitted Forms should also be saved in the wordpress backend
