Customize RS form in Joomla

I want to either: 1) Build an RS Form with a custom image background or 2) Have this form code pass the date attribute in m/d/y format <form action="/t3" method="get">Name: <input name="date" type="date" /><br /> Nights: <input name="nights" type="number" />Adults:<input name="adults" type="number" /><br /> <input type="submit" /></form> Even though I have the configuration set to m/d/Y, it passes the date as Y/m/d. There must be a format attribute to pass, but I cannot find the correct structure. The bottom line is that I want to have a form pass the date as m/d/Y, and have a specific image background. So that the form sits over the image and can be put in a module position.
