Custom Wordpress Zip/ Postal Lead gen Forms

We drive leads to our company across North America thru our website to our local dealers. We use WordPress and need you to help develop a tool and or use zapier that does the following: - Customer will input a postal code(6 digits or letters combos) or zip code (5 digits) that tells us where they are located. This will be a lead for our Dealers to follow up with. - Zip and postal code databases will be provided. They will be grouped and attached to an "area" known as a dealer. - Once a zip code is submitted (step 1) and they push enter, we need to open up more form fields that ask for name, address, phone, email, Notes (step 2). - When step 2 is completed, we need the lead forwarded to multiple paths (See attachment): 1. direct to the dealer via email with a CSV file attached. The lead can be in text format inside the body of the email. 2. Into Google Sheets onto specific sheets 3. OUr recommended 3rd party Jobber CRM (zapier approved) - The form will be placed in many places on our site. We need a way to be able to place the lead submit box where ever we need to.
