Convert interactive webform to Excel/Google Sheets

I have an interactive webform (example: [login to view URL]) that I need to be converted into a file type, preferably MS Excel or Google Sheets (whichever is your specialism). The actual form, used to capture Project Requirements, will have up to 100 fields. A rudimentary (unprocessed) Excel file will be available on project initiation. The final version will need a layout/worksheet that is optimised (vertically and horizontally) for printing outputs from all workbooks. Your use of simple (noncomplex) scripts will need to ensure both functionality (fit for purpose) and usability (operable by administrators). You should specify your interest, specialism (MS Excel or Google Sheets), and indicate very clearly if you have or have not previously completed this same (indistinguishably similar) type of project request. If you declare that you have, you will need to provide a sample of your work that you believe is a close fit; otherwise, you will need to provide an adapted sample, layout, or sketch for your intended output.
