Htmlize a page with jquery will be used for fb app

Hi, i need a master htmlizer, the person should be an expert in html jquery, and java script, it's a simple page but i need you to show 4-5 items for the cart, all buttons should be pressable, when a user clicks complete my order the form with email etc etc opens. the categories should be expandable and collapsable, the category that is opened will be black with the arrow up, the one that is not will have the arrow down, when we press "Bring me food as soon as possible" two extra options will open this should be a responsive design, should be super fast loading. put 5 -6 items in each menu so its easier for the programmers to understand, when clicked a popup should open (psd provided) and it will have radio and other buttons. each box will have instructions like Name box will have "Enter Your Full Name" Number will have "Enter Your Phone Number" Email address" Enter Your Email Address Delivery Address: Enter your full address, street, unit, apartment/house or office number, floor and nearest landmark, Need all css/color/jquery effects, but it should be instant and not have any lag.
