Simple website build

Build and upload website I am looking for a simple website to be created. The homepage is to serve as a sign up page for clients. We will provide a bespoke image to serve as a background image and a logo to appear on the page. The text 'WhatsEye' show appear adjacent to logo (this is the name of the service) The sign up process will require the following information to be collected: Clinicians full name Practice name Practice post code Mobile number Upon submission of a fully completed form a confirmation page will appear. Site navigation will appear at the bottom of the page and will include navigation to’ terms and conditions’, ‘contact us ‘ ‘about the service’ ‘signup’ The inputted data will be exported as a .vcf file and automatically delivered to a specified email address We will require the website to be uploaded to a hosting service of our choice. We are looking for the designer to produce a clean attractive website that does not detract from the background image, ideally the input boxes and text will be a white or pale colour to avoid the background image from being lost. The project is most likley to be awarded to a designer who can demonstrate the ability the come to us with creative ideas rather than us needing to micromanage to design process. A higher resolution background image can be provided if needed. The logo can be provided in different formats if required.
