Quick Minimalistic Bottle Label

Just need a quick minimalistic label that we are producing to put on some premade cocktails that we are bottling for Christmas presents. Prefer a 24-48 turnaround, more work available if the designer does a good job.
Sticker Label with be:
- 80mm x 50mm
- Preferably just black and white
- Matt Finish as we will be writing on the bottles
- The Elementum Logo needs to be front and center of the label
- Under the logo we need two lines for writing by hand the type of cocktail (the top line will nearly have to be 40mm with the bottom line 32mm) and centered
- Would prefer a border around label as shown in examples
- Also area for:
- batch number and bottle number with a small line to write for both
- Volume: 200mls/ 2 Cocktails
- Somewhere on the label I would like it to say Handmade Small Batched Cocktails
Artwork for logo and example are provided.
